In today's Weekly Digest we focus on Rule #4 of the Four Rules: Daily Exercise. For me this is the most challenging of the Four Rules, it just seems so difficult to include daily exercise into my over-scheduled schedule. But there is something so basic, so easy, so inexpensive that we can do daily and it will completely change our metabolism, our mood, our weight loss results. That thing is walking. Study after study confirms that a little walking goes a long way to improve our health and weight management efforts. Today's Digest is full of information and inspiration to get us moving, one step at a time, in the right direction. I am already excited to get out and get walking!

I Fall Flowers hope this newsletter finds you warm and well this second week of October. Thanks for spending time with me in our monthly 5DPT Bulletin.

Today we continue our discussion of the Four Rules - we are at Number 3: No Snacking. It's a tough one and I dare say most of will or have struggled with snacking following weight loss surgery. And, as you will see from the articles in this newsletter, not all bariatric centers follow the same Four Rules that include no snacking. But what is consistent, across the front lines of those of us living with weight loss surgery, is that out-of-control snacking on poorly chosen foods leads to a stall in weight loss and may possibly lead to weight gain. So please, take a look at the information here and revisit the information you were provided at the time of your surgery. Find your personal position on the "No Snacking" rule based on knowledge, experience, and environment -- it is the most empowering thing you can do for yourself in this ongoing battle of weight management in a world where it is much easier to be fat.

Neighborhood - Do you want it back?

Hello all you FRIENDS of LivingAfterWLS. I need some help here.

My company, LivingAfterWLS, needs to decide by Tuesday if we bring the LivingAfterWLS Neighborhood back or say goodbye our Safe Haven Circle of Friends online community forever.  Please respond to this post "Like" if you would like to see it back and if you would participate in the community. 


Just a point of history - the Neighborhood was started in 2006 and thrived for many years. The Neighborhood is an Invision Power Board powered online support community for weight loss surgery support and empowerment and membership requires registration.  The Neighborhood was a safe place to separate WLS discussions from the "Full Monty" of our life so often displayed on bigger social sites. Lasting friendships were established in the Neighborhood. In fact - many of you who connect here on FB met and made friendships in the Neighborhood. We enjoyed an active and supportive 5 Day Pouch Test forum as well as forums for all procedures, surgeries, and a multitude of interests. Membership exceeded 10,000 from around the world.

However we lost participation during the early wave of bigger social media destinations like FB and Twitter. We also noticed a remarkable trend that people tend to move on from WLS networking at about 3 years post-op, weary of the topic or frustrated with the hard work of losing weight "the easy way". I am fully aware some people left the Neighborhood as they took exception to me personally or my administrative decisions. I also know there were clicks forming and negativity festered in many threads. Please don't post or message me on these issues: I will not respond. 

Our community has been offline for myriad technical difficulties including spammer registrations and hostile attacks by hackers earlier this year. I have struggled with my own indecision. It is personally exhausting to manage an online community from a tech standpoint, an interpersonal communication management standpoint, and weariness of the WLS topic. Job burnout, don't ya know?  But I also know the valid and vital role it played in so many lives and I know it was a good and worthy online meeting place for like-minded people wanting healthier lives empowered by WLS and the strength and support of one another.

So what do you think? Bring back the Neighborhood or say goodbye forever?

Grilled Trout - Perfect Protein

How lucky am I? Jim served me this perfect WLS protein for breakfast. A fresh fillet of grilled Rocky Mountain Trout.  A 3-ounce serving of grilled trout provides 22 grams of protein to 4 grams fat and zero carbohydrate. Jim seasoned this with "Hot Shot" pepper blend and a pinch of kosher salt. He grilled it on the outdoor gas grill (thanks for not stinking up the house honey!) for about 4 minutes per side and served it to me with a dollup of plain yogurt blended with pickle relish for "Tarter Sauce" --- delicious and this meal kept me energized all morning.

I've got to say - this is one of my most favorite ever non-breakfast breakfast meals!

Thanks for joining me for the September 5 Day Pouch Test Bulletin, our first of the fall season. For me, now seems like the right time to review my understanding of body hunger and remind myself of the difference between hunger and appetite. In hearing from you in the weight loss surgery communities I know that many of us struggle time and again against hunger -- or what we think is hunger.  One of the best breakthroughs in understanding my food relationship came in 2008 when I read the words, "Hunger is not an emergency." My goodness! Who knew? Read my blog post account of that enlightening moment:
"Just this week I had a breakthrough moment when I read that naturally slender people do not treat hunger as an emergency. "Most of us who struggle with extra pounds tend to view hunger as a condition that needs to be cured - and fast," writes Judith S. Beck, PhD, author of the Beck Diet Solution. "If you fear hunger, you might routinely overeat and avoid it," she says adding, "Thin people tolerate hunger because they know hunger pangs always come and go, buying them some time."
Hunger defined: the painful sensation caused by a lack of food that initiates food-seeking behavior.
Hunger is not an emergency. Interesting, don't you think? Since publishing the 5DPT I've received tremendous feedback. Some people are amazed to not feel hungry, even on those difficult first two days. Others report "climbing the walls" hunger. I believe each of us responds differently to the 5DPT and there are certainly extremes between lack of hunger and ravenous hunger."

I have this book and live by it. Kaye Bailey is an advocate for those of us that have had weight loss surgery. She has written this book with love and experience. When I get off track from weight loss surgery and feel hopeless, I pick up this book and follow the five day pouch test. In those five days I feel back in control and empowered. Another plus to doing the five day pouch test is that I usually lose a few pounds as a bonus. I highly recommend this book to anyone who has had WLS. I have bought several of these books as gifts for my new weight loss surgery friends.

Stawberries & Asparagus
Including fresh vegetables and berries in a meal of lean protein assures you get a variety of nutrients and fresh flavor without sacrificing your healthy living goals. Some weight loss surgery patients notice the stringy fiber of asparagus causes digestive discomfort. Look for tender young stalks to avoid the stringy fibers or use a vegetable peeler and remove the outer layer of each stalk to enjoy the tender asparagus without the stringy fiber.

Lemony Asparagus with Parsley
Serves 4

Ingredients:
1 teaspoon olive oil
1/2 pound fresh asparagus, trimmed and sliced into 2-inch pieces
2 tablespoons finely chopped fresh flat-leaf parsley
zest of 1 lemon
1/2 lemon, juiced
1/4 teaspoon salt
1/4 teaspoon ground black pepper

Directions:
Heat oil in a large nonstick skillet over medium-high heat. Add asparagus; cook, stirring occasionally, 7 minutes or until crisp-tender. Remove from heat. Stir in chopped parsley, lemon zest, lemon juice, salt and black pepper. Serve warm.

Per 1/2-cup serving: 35 calories, 3g protein, 2g fat, 5g carbohydrate and 2g dietary fiber.

A major area of concern and sometimes confusion for us after weight loss surgery is how many grams of carbohydrates should we eat in a day. Have you been perplexed over the carbohydrate question? Today we take a look at carbs and the role they play our diet after weight loss surgery.

It is popular in the health and nutrition fields to describe carbs as "good carbs" or "bad carbs". I prefer not to use those words because they assign a moral trait to food and food does not have moral traits. Food is food. When we remove moral assignment from the food we eat we can be smart, thoughtful, and rational with our choices. But if we assign morals to food and eat "bad carbs" then we must be bad. You know the self-blame cycle I'm talking about.

Instead of good carbs and bad carbs I prefer to use more accurate terms:

  • Fruit and Vegetable Carbs or Complex Carbs
  • Grain and Starch Carbs or Complex Carbs
  • Processed Manufactured Carbs or Simple Carbs
I have them arranged in nutritional importance to us after undergoing weight loss surgery. Like other nutrients carbohydrates are measured in grams. A 2008 broad-canvas study of bariatric centers revealed that few bariatric surgeons or nutritionists give a specific daily quota for carbohydrate intake. Most default to the 113g RDA recommended by the National Academies' Institute of Medicine for people following a 1200 calorie a day diet. It takes roughly 8 to 10 cups of fruit and vegetables to equal 113g complex carbohydrates and I do not know any weight loss surgery patient who can consume that much volume.

That is good news. While it is unlikely we can ever reach that daily intake by eating fruits and vegetables it is also unlikely we will ever over eat our carbohydrate allowance when we select high moisture, high fiber fruits and vegetables. A variety of fruits and vegetables promotes good health because they contain disease fighting vitamins and nutrients. Fruits and vegetables enjoyed with lean protein improve the overall eating experience for weight loss surgery patients because they are high in moisture. This is a relief when we are following the liquid restrictions and not drinking a beverage with meals.

Today we look at Protein & Fruit WLS Perfect Mealcomplex carbohydrates from fruits and vegetables. We have some great tips and recipes to include them in your diet for life-long weight management. Pictured here is a typical breakfast plate for me that included 2 ounces white meat roasted turkey, 1 hard-cooked egg, 6 blackberries and 1/2 large kiwifruit. After enjoying this meal I will not feel hungry or crave sweets for several hours. Nutritional Count: 182 Calories. (44% Protein, 32% Fat, 24% Carbohydrate); 20g Protein; 6g Fat; 11g Carbohydrate; 3g Dietary Fiber. 75% RDI Vitamin C, 25% RDI Niacin, 5% RDI Calcium.) I consider this plate a perfect breakfast and while I don't get it right every morning it is a goal I aim for because on days when I get this right I am powerful and in control.

What's in your water?

Navigating the rough waters of beverage labeling

Morbidly obese people that undergo bariatric surgery for weight loss are instructed to drink lots of water as part of their post-surgical dietary plan. In fact, "Drink Lots of Water" is the second of Four Rules most bariatric surgeons require of patients following any gastric surgical procedure for weight loss. The Four Rules are essential behaviors to be followed during the early months and weeks following weight loss surgery during which weight loss occurs. For life-long weight maintenance patients will continue to follow the Four Rules once goal weight is achieved. In addtion to drinking lots of water patients will follow a high protein diet, avoid snacking and engage in daily exercise.

Water makes up about 60 percent of the adult body's weight and is essential for the transport of nutrients and waste products throughout the body. When a person drinks an adequate amount of the correct type of water it may have a positive effect on their health. The first sign of inadequate fluid intake is thirst and symptoms of dehydration may soon follow.

Weight loss surgery patients who struggle to drink enough water often ask if other beverages count toward their total fluid intake. The advice from bariatric nutritionists varies, but in most cases patients learn the cleaner their fluid intake the more favorable health outcome they will achieve. Below is a glossary of terms one may encounter when purchasing water. A basic knowledge of these terms is useful in helping achieve good body fluid balance.

Artesian water is drawn from a well that taps a confined aquifer in which the water is under pressure.

Bottled water is drinking water sold in bottles.

Carbonated water contains carbon dioxide gas, either naturally occurring or added, that causes bubbles to form in it; also called bubbling or sparkling water. Seltzer, soda, and tonic waters are legally soft drinks and are not regulated as water.

Distilled water has been vaporized and recondensed, leaving it free of dissolved minerals.

Filtered water is treated by filtration, usually through activated carbon filters that reduce the lead in tap water, or by reverse osmosis units that force pressurized water across a membrane removing lead, arsenic, and some microoganisms from tap water.

Hard water has a high calcium and magnesium content.

Mineral water comes from a spring or well that typically contains 250 to 500 parts per million (ppm) of minerals. Minerals give water a distinctive flavor. Many mineral waters are high in sodium.

Natural water is obtained from a spring or well that is certified to be safe and sanitary. The mineral content may not be changed, but the water may be treated in other ways such as with ozone or by filtration.

Public water is from a municipal or county water system that has been treated and disinfected.

Purified water has been treated by distillation or other physical or chemical processes that remove dissolved solids. Because purified water contains no minerals or contaminants, it is useful for medical and research purposes.
Soft water has a high sodium or potassium content.

Spring water originates from an underground spring or well. It may be bubbly (carbonated), or "flat" or "still," meaning not carbonated. Brand names such as "Spring Pure" do not necessarily mean that the water comes from a spring.

Well water is drawn from ground water by tapping into an aquifer.

Liquids Restrictions & Liquid Meals: Making sense of it

Recently I have observed some confusion in our online communities about the weight loss surgery liquid restrictions. Reason for the confusion are many including different instructions from different doctors and different interpretation of the liquid restrictions by patients.  Sometimes I get confused about the rules when it comes to consuming liquid with meals.

The Four Rules of WLS

For clarification let's note that the professional consensus is people who have undergone any of the currently approved bariatric procedures for weight loss should not drink liquids while eating meals. A meal is considered any food (liquid, soft protein, firm protein, complex carbs., etc) that contains nutrients and is meant to fuel and satiate the body. More often than not patients are advised to avoid liquids for a specified time period before and after eating a meal. This window of time is called by many the "Liquid Restrictions" and some people note it this way: 30/15/30  or 15/15/15. In the first example there would be no liquids 30 minutes before a meal, no liquids during a timed meal of 15 minutes, and no liquids for 30 minutes following the last bite of a meal. If you are unsure of your specific liquid restrictions check with your bariatric center nutritionist, in your dietary guidelines, or with your support group leader.

Now, I do not personally sit on a stop watch while fasting from liquids, but I do stay mindful of the rule. It is important for two reasons: first the pouch or sleeve will stay full longer if the food is allowed to digest before being washed through with liquid. Secondly, the longer the food is allowed to travel through the digestive system without being pushed by liquids, the more nutrients will be observed. When we are eating so few calories, because of the surgery, it is highly important that we give our body ample time to make the most of what we feed it.

Liquid restrictions should be followed when eating a liquid meal such as a protein fortified smoothie or one of our favorite 5 Day Pouch Test Soups.  We want to feel full and satisfied longer which will prevent snacking after meals, and we want to enjoy the benefits of the nutrition we provided our body in the meal.

Restrict Liquids, Drink Lots of Water - HUH?

In a country where food and drink go hand in hand weight loss surgery patients are challenged to follow the liquid restrictions as instructed by their bariatric center. But understanding and following the liquid restrictions plays a key role in long-term weight maintenance following gastric bypass, gastric sleeve, or adjustable gastric banding (lap-band) surgeries.

In general, bariatric centers instruct weight loss surgery patients to avoid drinking liquids for thirty minutes before a meal, during the meal, and for thirty minutes following the meal. This easily adds up to 90 minutes of no liquids three times a day: four-and-one-half waking hours. It is easy for patients to become frustrated with these restrictions because another rule of weight loss surgery is to drink lots of water, at least 64 ounces a day.
